Roles & Responsibilities
-

Coordinate and facilitate the complete transplant process from patient evaluation through post-transplant follow-up.

Act as the primary point of coordination between patients, their families, doctors, nursing teams, and other hospital departments.

Coordinate donor and recipient evaluation, investigations, appointments, and required documentation.

Ensure timely scheduling and coordination of consultations, diagnostic tests, procedures, and transplant-related assessments.

Maintain accurate patient and transplant-related records, reports, and documentation.

Coordinate with multidisciplinary teams including transplant surgeons, physicians, anesthetists, nursing, diagnostics, blood bank, ICU, and other support departments.

Assist in coordinating pre-transplant and post-transplant patient care and follow-up requirements.

Ensure patients and their families receive timely information regarding appointments, procedures, documentation, and transplant-related processes.

Coordinate with relevant departments to ensure smooth patient movement and minimize delays in the transplant pathway.

Follow up on pending investigations, reports, approvals, and other transplant-related requirements.

Maintain confidentiality of patient information and ensure compliance with hospital policies and transplant-related protocols.

Support the transplant team in maintaining databases, trackers, reports, and MIS related to transplant cases.

Coordinate with external agencies/authorities wherever required for transplant-related documentation and approvals.

Handle patient and family queries professionally and escalate clinical or operational concerns to the appropriate team.

Ensure effective communication and coordination throughout the transplant journey to provide a smooth patient experience.
