Key Responsibilities
- Conduct regular medical assessments of patients and monitor their overall clinical condition, vital signs, symptoms, and recovery progress.
- Manage and provide medical care for post-hospitalisation, post-stroke, rehabilitation, and medically dependent patients.
- Review patients' medical history, hospital discharge summaries, investigation reports, medications, and ongoing treatment plans.
- Prepare, review, and update individual treatment and care plans based on the patient's current medical condition.
- Monitor patients for any changes in their condition and take appropriate medical action when required.
- Conduct regular patient reviews and document clinical findings, progress, and treatment recommendations.
- Coordinate closely with nursing staff, patient care assistants, physiotherapists, and other members of the care team to ensure continuity of patient care.
- Provide clear medical instructions to nursing and care staff regarding medications, monitoring, feeding, positioning, wound care, catheter care, tracheostomy care, and other patient-specific requirements, as applicable.
- Review and monitor patients requiring tracheostomy care, feeding tubes, catheter care, wound care, oxygen support, and other post-hospital care, according to their individual treatment plans.
- Coordinate with external consultants, specialists, diagnostic centres, and hospitals whenever specialist consultation, investigation, or hospitalisation is required.
- Evaluate patients during medical emergencies, initiate appropriate immediate management within the scope of practice, and coordinate hospital transfer or emergency care when necessary.
- Monitor medication schedules and review prescriptions, medication changes, possible side effects, and treatment compliance.
- Review laboratory and diagnostic investigation reports and take appropriate clinical action or communicate findings to the concerned consultant.
- Maintain accurate and up-to-date medical records, patient notes, treatment plans, investigation reports, medication records, and clinical documentation.
- Communicate regularly with patients and their family members regarding the patient's medical condition, progress, treatment plan, and care requirements.
- Explain medical instructions and precautions to family members and ensure that important changes in the patient's condition are communicated appropriately.
- Participate in multidisciplinary discussions regarding patient care and recovery.
- Monitor patients' nutritional, functional, and rehabilitation progress in coordination with the relevant care and rehabilitation team.
- Identify potential complications at an early stage and take appropriate action or escalate the matter to the concerned specialist/hospital.
- Conduct periodic medical rounds and review patients according to the centre's clinical requirements.
- Ensure that medical orders and treatment instructions are properly communicated and followed by the nursing and care team.
- Support the centre in maintaining appropriate clinical standards, patient safety practices, infection-control practices, and documentation procedures.
- Provide guidance and clinical support to nursing staff and patient care teams whenever required.
- Assist in coordinating planned hospital visits, investigations, follow-up consultations, and transfers.
- Maintain professional and respectful communication with patients, relatives, staff, consultants, and hospital teams.
- Ensure timely escalation of any significant deterioration or medical concern to the appropriate specialist or hospital.
- Perform any other clinical responsibilities reasonably related to the role and patient-care requirements of the rehabilitation centre.
