Job Summary

The Senior Staff Nurse specializing in geriatric care is responsible for providing clinical leadership, direct nursing care, and coordination of healthcare services for elderly residents within the assisted living facility. They oversee the delivery of nursing care, mentor junior staff members, and collaborate with interdisciplinary team members to promote the health, safety, and well-being of older adults.

Key Responsibilities

Clinical Leadership and Supervision

  • Provide clinical leadership, guidance, and supervision to nursing staff members, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs), ensuring adherence to standards of practice, policies, and procedures.
  • Assign and delegate nursing tasks, monitor workload distribution, and evaluate performance to optimize efficiency, productivity, and quality of care delivery within the assisted living facility.

Direct Nursing Care and Assessment

  • Perform comprehensive nursing assessments of elderly residents to evaluate their physical health, cognitive function, functional status, and psychosocial needs, utilizing standardized assessment tools and clinical judgment.
  • Develop individualized care plans, establish nursing diagnoses, and prioritize interventions based on assessment findings, resident preferences, and interdisciplinary team input.

Medication Management and Administration

  • Administer medications, treatments, and prescribed therapies safely and accurately, following established medication administration protocols, dosage calculations, and documentation procedures.
  • Monitor residents' response to medications, assess for adverse reactions, and collaborate with physicians, pharmacists, and allied health professionals to adjust medication regimens as needed.

Health Promotion and Disease Prevention

  • Promote health education, disease prevention, and wellness initiatives among residents, families, and staff members, addressing topics such as fall prevention, medication management, nutrition, and chronic disease management.
  • Conduct health screenings, immunization clinics, and health assessments to identify risk factors, monitor vital signs, and facilitate early detection of health problems in older adults.

Care Coordination and Interdisciplinary Collaboration

  • Collaborate with interdisciplinary team members, including physicians, therapists, dietitians, social workers, and recreational therapists, to develop and implement integrated care plans tailored to residents' individual needs and preferences.
  • Participate in interdisciplinary team meetings, care conferences, and resident care planning sessions to coordinate care transitions, monitor progress, and address residents' evolving healthcare needs.

Quality Assurance and Compliance

  • Ensure compliance with regulatory standards, nursing practice guidelines, and facility policies related to resident care, documentation, infection control, and safety protocols within the assisted living environment.
  • Conduct regular audits, assessments, and quality improvement initiatives to monitor the quality of nursing care, resident satisfaction with healthcare services, and adherence to established clinical protocols.

Qualifications and Skills

  • Registered Nurse (RN) licensure or equivalent nursing credential required.
  • Bachelor of Science in Nursing (BSN) degree preferred, with specialization or certification in geriatric nursing or long-term care nursing highly desirable.
  • 3- 8 Years of clinical experience working as a nurse in a geriatric care setting, with demonstrated proficiency in assessment, care planning, medication management, and interdisciplinary collaboration.
  • Strong leadership, communication, and team-building skills, with the ability to mentor, motivate, and empower nursing staff members to achieve clinical excellence and deliver person-centered care.
  • Advanced knowledge of geriatric nursing principles, age-related health conditions, and evidence-based interventions for addressing the unique needs of older adults.
  • Proficiency in utilizing electronic health record (EHR) systems, clinical documentation software, and nursing informatics tools to record, track, and analyse resident data and healthcare outcomes.
  • Commitment to continuous learning, professional development, and adherence to ethical standards and nursing best practices in the care of elderly populations.
